Medicare Supplement Plan G costs vary by insurance company and where you live. For a 65-year-old, the monthly premium can differ substantially even though Plan G's standardized benefits are the same from one company to another.

Your Plan G cost can also depend on your age, sex where permitted, tobacco use, available discounts, and how the insurance company rates its policies.

Below, you can compare sample Plan G costs by carrier and state, see what affects your premium, and learn how to compare Plan G prices correctly.

What Affects Your Plan G Cost?

Here's what we've found affects your Plan G premium:

  • ZIP code: Where you live can significantly affect your premium. Medigap rates vary by state and may also vary by geographic rating area within a state.
  • Age: Depending on how the policy is rated, which is often dictated by the state, your age may affect your starting premium and whether aging itself affects your premium in later years. Medigap policies can be community-rated, issue-age-rated, or attained-age-rated.
  • Gender: Some states and carriers price men and women differently; others don't.
  • Tobacco use: Some insurers charge higher premiums based on tobacco use, although the rules and impact vary by state and carrier.
  • Household discounts: Some carriers offer a premium discount if you meet their household eligibility requirements — see below.
  • Underwriting and eligibility: Outside certain enrollment protections, an insurer may require health questions before approving your application. The rules vary by state and situation, so a quoted price doesn't necessarily mean you've been approved for coverage.

Does a More Expensive Plan G Give You Better Coverage?

No.

We've found that in most states, Medicare Supplement plans with the same letter provide the same standardized basic benefits no matter which company sells them.

A Plan G from one carrier doesn't cover more Medicare-approved expenses just because its premium is higher.

That's why we recommend focusing on differences such as the premium, available discounts, rating method, customer service, and any additional services or perks — not differences in the standardized Plan G benefits.

How to Compare Plan G Prices

Here's what we look at when we compare Plan G prices:

  • Compare Plan G with Plan G. Make sure every price you're comparing is for Plan G and is based on the same personal information and discount assumptions.
  • Monthly premium. The starting-point number, but not the whole picture.
  • Discounts. Ask whether the quote already reflects a household discount or other adjustment.
  • Rating method. Ask whether the premium is community-rated, issue-age-rated, or attained-age-rated — this affects whether aging itself can affect your premium over time.
  • Company and financial strength. A rating from A.M. Best, Moody's, or S&P can tell you about the insurer's ability to meet its financial obligations, but it doesn't tell you which company will have the lowest premium or smallest future increases.
  • Extras. Some carriers offer additional services, such as fitness or wellness programs, that aren't part of Plan G's standardized insurance benefits. Availability varies, so we treat these as secondary to the premium and standardized coverage.

Want to compare the companies themselves? We've reviewed each of these carriers — see our Aetna Plan G, Cigna/HealthSpring, Humana, and Mutual of Omaha reviews for more information about each carrier.

Can Your Plan G Premium Increase?

Yes — we want you to know Medigap premiums can increase over time, even after you enroll.

Community-rated policies generally don't base the premium on your age.

Issue-age-rated policies base the premium on your age when you first buy the policy, while attained-age-rated policies can become more expensive as you get older.

Regardless of rating method, premiums can still increase for other reasons.

We'd caution that the cheapest quote today isn't a promise that a carrier will remain the least expensive in future years. Understand how the policy is rated and remember that premiums can increase after enrollment.

Do You Have to Answer Health Questions to Enroll?

It depends on timing.

Your six-month Medigap Open Enrollment Period generally begins the first month you're 65 or older and enrolled in Medicare Part B.

During this period, insurers generally cannot use medical underwriting to deny you a policy or charge you more because of your health.

Outside this period, an insurer may require medical underwriting before allowing you to switch policies. However, guaranteed-issue rights and additional state protections can allow some people to change coverage without medical underwriting.

Household Discounts

We've seen Medigap carriers offer a household discount that reduces your premium.

The percentage and eligibility rules vary by company and state — depending on the insurer, you may need to live with another adult or have multiple household members enrolled with that same carrier.

When comparing quotes, ask whether the displayed premium already includes the household discount, and compare the final rate you'd actually qualify for, not just the carrier's advertised base rate.

How to Get an Accurate Plan G Quote

Here's what we recommend before you request a quote:

  1. Have your ZIP code, date of birth, gender, and tobacco-use status ready.
  2. Get Plan G quotes from at least three carriers.
  3. Ask each carrier whether the quote reflects a household discount and what rating method it uses.
  4. Before applying, find out whether you have an enrollment protection or whether medical underwriting will apply.

Bottom Line

We've found that in most states, Plan G's basic benefits are standardized, so paying a higher premium to one insurance company doesn't get you more standardized Plan G coverage.

What we see differ is the premium, available discounts, rating method, customer service, and additional services or perks offered by the carrier.

That's why we always recommend comparing current quotes from multiple companies using the same personal information and accounting for any discounts you qualify for.

FAQ

In most states, no. Plan G's basic benefits are standardized, so a lower-priced Plan G doesn't provide fewer standardized benefits simply because it costs less.

Not necessarily. Depending on the carrier's rating method, your premium can rise with age, due to other rate increases, or both. Ask about the rating method before enrolling.

You may. Outside your Medigap Open Enrollment Period, an insurer may require medical underwriting before allowing you to switch policies. However, guaranteed-issue rights and additional state protections can allow some people to change coverage without medical underwriting.

Yes. You can compare Plan G premiums before deciding whether to apply. Getting a quote doesn't enroll you in a policy. Keep in mind that a quoted premium doesn't necessarily guarantee approval if medical underwriting applies.

In some cases, yes. If you itemize deductions and your total medical expenses exceed a certain percentage of your income (7.5% of adjusted gross income for most people), your Medigap premiums may be tax-deductible. Always consult with a tax advisor to understand your eligibility.
